CSL Biotherapies (NZ) Ltd ("CSL, we or us") understands the importance of safeguarding the privacy of information that we collect from our on-line visitors via the Internet. Because the Internet offers the ability to collect certain types of information, we have set out below the terms and conditions surrounding the capture and use of the information we gather.

Capture of Information:
Through the websites which CSL operate, we may collect two types of information about you:

1. Personal information which is provided voluntarily by you upon registration for the services that we offer. Normally, this information includes your first and last names, title, email address, location address and office telephone number. You may view and have this information modified by contacting us at nzadmin@csl.co.nz. From time to time, CSL may collect additional personal information from users of the website in connection with optional surveys, news updates or promotions that you are asked to participate in through the website. Providing this information is optional. Feedback from surveys will be used to improve the website.

2. Compiled information generated by our systems or third party systems we use, which track traffic, but which does not relate to you personally.

We only use your personal information to:
Help us to provide our products and services;

Improve our service;

Inform or keep you updated on products or services you have indicated an interest in;

Inform or update you about additional services.

Disclosure of Information:
CSL may disclose your personal information to third parties:

When required to do so by law; or

After seeking your prior permission before doing this. Permission is usually obtained at the time of collecting the information from you.

We will not use your personal information without taking such steps as are reasonable in the circumstances to ensure that the information is accurate, up-to-date and not misleading.

Personal information collected by CSL may be transferred to other websites of CSL where it is necessary for the purposes for which you submitted the information. By submitting personal information on CSL's website, you consent to transborder transmission of information provided that such information remains in the control of CSL.

Cookies and Tracking:
We may, via the website, set and access "cookies" on your computer to provide assurance that our proprietary content and third party content is only accessed by users with appropriate authorisation to the website. Many browsers recognise when a cookie is offered, and allow you to decide whether to accept it. The cookie file is completely anonymous and provides no personal information about website users. Additionally, all stored user information is protected from unauthorised access through the use of secure passwords, user log-ons and other security procedures.

If any collection, use or disclosure of your personal information is outside the regulations set out in this Privacy Policy, we will provide you with the option to either:

Elect not to receive future similar communication from CSL; or

Elect to continue to receive future similar communication from CSL.

The CSL website may contain links to other websites. CSL is not responsible for the privacy policies of such other websites. This privacy policy applies solely to information collected on the website by us.

Duration of storage of personal information:

CSL will only retain your personal information for as long as necessary to fulfil the purposes for which the information was collected or as required by law.

Your personal information is kept on our own computer and is totally secure. You have the right at any time to have access to, and update, correct or remove the information about you that we hold. *Girls born in 1990 or 1991 who have started their vaccine course before 31 Dec 11 will still be eligible for FREE vaccination on their remaining shots. Girls born on or after 1 January 1992 will continue to be eligible for FREE vaccination. Please refer to the Ministry of Health for the eligibility criteria for girls younger than 11. A patient charge applies for all others outside this range.

Immunisation does not replace the need for regular cervical smear testing.

Gardasil [Quadrivalent HPV (Types 6, 11, 16, 18) Recombinant Vaccine]. Prescription Medicine. Indication: GARDASIL is a vaccine that helps prevent the following diseases in girls and women 9 through 45 years of age caused by Human Papillomavirus (HPV) Types 6, 11, 16 or 18: cervical cancer (cancer of the lower end of the uterus or womb); vulvar (the outside of the female genitals) and vaginal cancers; abnormal and precancerous cervical lesions (changes in cells of the cervix which have a risk of turning into cancer) as found by a Pap test; genital warts and HPV infection. You cannot get any of these diseases from GARDASIL. Contraindications: Do not, or do not let your child have GARDASIL if you or your child has an allergy to any of the ingredients. If you are not sure whether you or your child should be given GARDASIL, talk to your doctor or health care provider. Warnings and Precautions: Tell your doctor if you or your child has had an allergic reaction to the vaccine; has a bleeding disorder and cannot receive injections in the arm; has any illness with a fever higher than 37.8°C; is pregnant; is planning to get pregnant; has a weakened immune system; takes or plans to take any medicines including ones you buy without a prescription. Adverse Events: For the most part, the following common side effects for GARDASIL have been mild: pain; itching; tenderness or soreness; redness; swelling; fever; dizziness; nausea and vomiting. Although uncommon, fainting sometimes accompanied by shaking or stiffening has been reported. As with other vaccines, side effects that have been reported during general use include: Swollen glands in the neck, armpit and groin; joint pain; aching muscles; unusual tiredness, weakness or confusion; chills; generally feeling unwell; allergic reactions; bleeding or bruising more easily than normal; skin infection (with hot, tender and red skin). If any of the above symptoms worsen or you or your child has any unusual or severe symptoms after receiving GARDASIL, contact your doctor or health care provider right away. Tell your doctor if you are using, or have recently used any other medicines, including those for which you needed no prescription. Ingredients: The active ingredients of GARDASIL are highly purified inactive proteins from HPV Types 6, 11, 16 and 18.
